Neurontin’s active ingredient, gabapentin, is not structurally related to GABA (gamma-aminobutyric acid), despite its name. Instead, it modulates voltage-gated calcium channels in neuronal membranes, specifically binding to the alpha-2-delta subunit. By doing so, Neurontin reduces the release of excitatory neurotransmitters such as glutamate, norepinephrine, and substance P. This action dampens abnormal nerve firing associated with neuropathic pain and some forms of epilepsy.
